Foreigner Physical Examination Form
The Foreigner Physical Examination Form must be completed by a licensed physician at an authorized hospital. The form must be fully filled in and include the attending doctor’s signature, the official hospital stamp, and a passport-sized photo of the applicant stamped by the hospital. Incomplete forms, or forms missing any of these elements, will be considered invalid. Please note that the medical examination results are valid for 6 months only, so applicants should ensure the examination is conducted within the required time frame.

Beijing Municipal Government Scholarship Application Form
Applicants who wish to apply for scholarships must complete and upload the Beijing Municipal Government Scholarship Application Form as part of their application. This document is required only for scholarship applicants and must be submitted together with the other application materials.

Curriculum Vitae (CV)
A CV including a chronological list of educational experiences starting from the undergraduate level. Applicants with prior study experience in China must include all Chinese study experiences.
Foreigner Physical Examination Form
A completed Foreigner Physical Examination Form issued by an authorized medical institution. Please note that the form will be considered invalid if it is incomplete or missing: The attending physician’s signature The hospital’s official stamp The applicant’s photo or official seal The physical examination results are valid for six (6) months from the date of issue.
Non-Criminal Record Certificate
An official Non-Criminal Record Certificate issued by the local police authority, confirming that the applicant has no criminal history. This document is mandatory for admission and residence procedures.
Passport and Chinese Visa Page
Applicants must submit a scanned copy of a valid passport (photo/information page). Applicants who have previously studied in China or are currently residing in China must additionally provide a scanned copy of the Chinese visa page or residence permit page. These documents are used for identity verification and immigration status review.
