Foreigner Physical Examination Form
The Foreigner Physical Examination Form must be completed by a licensed physician at an authorized hospital. The form must be fully filled in and include the attending doctor’s signature, the official hospital stamp, and a passport-sized photo of the applicant stamped by the hospital. Incomplete forms, or forms missing any of these elements, will be considered invalid. Please note that the medical examination results are valid for 6 months only, so applicants should ensure the examination is conducted within the required time frame.
A recent bank statement demonstrating sufficient financial capacity to support tuition fees and living expenses during the study period. The minimum required amount is USD 2,500 or equivalent. The statement must clearly show the account holder’s name and balance.
A list of papers published in professional or scientific journals, representative papers, or abstracts of academic articles, if available. These materials are optional, but may strengthen the application and demonstrate research capability.
